Important information:

In Type-1 diabetes, the body completely stops producing any insulin. Insulin is usually produced by cells in the pancreas and these cells are dysfunction in type 1 diabetes due to an attack by the person’s own immune system. Therefore patients with type 1 diabetes must take daily insulin injections to survive. This form of diabetes usually develops in children or young adults, but can occur at any age. It is relatively less common as compared to type 2 diabetes.
Common thinking is, Type-1 diabetes is caused by genes and environmental factors, such as viruses, that might trigger the disease.It occurs when your immune system, the body's system for fighting infection, attacks and destroys the insulin-producing beta cells of the pancreas.
People who have type-1 diabetes must take insulin as part of their treatment. Because their bodies can't make insulin anymore, they need to get the right amount to keep their blood sugar levels in a healthy range. The only way to get insulin into the body now is by injection with a needle or with an insulin pump.There is no cure for diabetes, but it can go into remission. Important information:

Diabetes diagnosed for the first time during pregnancy (gestation) is Gestational diabetes. Gestational diabetes mellitus (GDM) is defined as any unusuallyhigh glucose level diagnosed with onset or first recognition during pregnancy. Depending on individual case, it is decided whether insulin or only diet modification is used for treatment.Whether or not the condition persists after pregnancy depends on many genetic and environmental factors.
During GDM, the placenta makes hormones that cause glucose to build up in mother’s blood. It can increase the mother’s blood sugar that can complicatethe pregnancy and the baby's health.Gestational diabetes raises the mother’s risk of high blood pressure, as well as, preeclampsia which is a serious complication of pregnancy that causes high blood pressure and other symptoms that can threaten the lives of both mother and baby.

Steps for a healthy pregnancy

1. It is recommended that the pregnant mother should exercise regularly three days a week for 30 minutes.
2. She should not skip meals. Aim to eat every two hours. The blood sugar levels should be regulated by eating a healthy snack or meal every two hours.
3. The pregnant mother should take her prescribed vitamins and other medication regularly.
4. Visit the clinic as may be advised.

Patients with diabetes commonly have musculoskeletal problems (diabetes hand, frozen shoulder, carpal tunnel syndrome, and others). We screen, identify and manage these conditions which produce considerable morbidity. These are seen in both type 2 and type 1 diabetes. In addition, since type 1 diabetes is an autoimmune disorder, there is a chance of overlap with other autoimmune diseases like arthritis, celiac disease, lupus. Stress aggravates diabetes. It raises blood sugar levels, activates fat cells, impairs glucose tolerance, increases insulin resistance and impacts blood pressure.Vice versa, people with Diabetes have 30% higher chance of stress related disorders. Presence of stress interferes with control of sugar; there may be increase in dose of medicines, increased chances of medical complications. The Clinic is run by a team of psychiatrist and clinical psychologists. The clinic offers evaluation for psychological issues with medical treatment and counselling for stress management.

Important Information:

Diabetes unit is equipped with high quality diagnostic and screening equipment to screen and measure risk factors of Diabetes. To name a few:DEXA, Periscope, ABI, Trinetra test,EZ scan, Biothesiometry, ABPM. These tests are done as and when required, and their schedule is as per appointment.

At our clinic, the tests done are:

DEXA (Dual-energy X-ray Absorptiometry): Dexa is used to measure bone mineral density or Bone mineral content for screening & diagnosis of osteoporosis. This helps in identifying those who are at high risk for fracture which can be treated. It is particular useful in chronic diseases like chronic kidney disease, diabetes etc. Additionally, it also measures body composition (fat %, lean % of the body) which has changed the perspective of measuring obesity. KEM Diabetes unit has pioneered the use of body composition in Indians. Approximately, 4-5 patients are tested per day.
Cardiovascular risk profiler (Periscope): Periscopeis again a Novel method of measuring cardiovascular health in the community by measuring Pulse wave velocity. It’s a non-invasive method to measure arterial stiffness which is highly reproducible and can predict future risk of cardiovascular event.
Peripheral Doppler to measure Ankle Brachial Index (ABI): ABIis an important measure to diagnose peripheral vascular disease. The index is important and compares the blood pressures of the arms and legs to give a ratio that can suggest various severity of peripheral vascular disease. About 4-5 patients are screened every day.
Trinetra test: Ituses highly specialised equipment called Trinetrato record and store photograph of the Fundus. Diabetic retinopathy is one of the important micro vascularcomplications of diabetes. The equipment empowers physicians to capture minute changes in the fundus for efficient treatment. On an average 1-2 patients undergo this test every day.
EZ scan also called SUDO scan: It isa non-invasive highly sensitive and reproducible tool for the early detection of peripheral neuropathy. It helps to screen and measure nerve conduction for better treatment to control complications caused by diseases like type-2 diabetes. This test is conducted about once or twice per week, as per incoming patients.
Biothesiometry: It is a simple and economical tool for measurement of Vibration Perception Threshold. It is predominantly used for foot care to see understand sensitivity of the foot (sensitivity threshold) and to vibration diagnosing peripheral neuropathy. 4-5 patients are screened per day as per the diabetes package.
Ambulatory Blood Pressure Monitoring(ABPM): ABPMis accomplished with a special device that records blood pressure periodically (usually at 15-minute or 30-minute intervals for either 24 or 48 hours) during routine daily activities and while sleeping. Periodic readings help to decide whether a person actually has hypertension over blood pressure readings taken in the doctor’s office. (White coat hypertension). This test is done very rarely. Very few cases 2-3 per month require to undergo this test.

Regular and enough good quality sleep is of vital importance to maintain healthy physical and mental health. Sleeping is like getting your body recharged for the next day. OSA (Obstructive sleep apnoea) is the commonest breathing disorder during sleep.Obstructive sleep apnoea is a repeated episodes of partial or complete collapse of the airways causing intermittent hypoxia during the sleep. This Leads to disruption of normal sleep resulting into excessive daytime sleepiness to compensate for the lost sleep in the night. It may also lead to variety of symptoms starting from morning headaches, reduced cognitive performance, resistant hypertension, vehicle accidents during one of the irresistible naps during driving. Sleep disordered breathing can be diagnosed with overnight polysomnography or sleep study.
Sleep study or polysomnography can be conducted either in hospital or at home. It is a non-invasive overnight examination which monitors thoracic and abdominal respiratory efforts, nasal flow, ECG, EEG, oxygen saturation and body movements throughout the sleep. It also has a microphone to record snoring. The data collected digitally in the overnight sleep is analysed by the sleep specialist next day. After analysis of data collected, sleep specialist can give a diagnosis of the type of sleep disordered breathing and guide treatment.
Moderate to severe severity of OSA can be treated with CPAP (Continuous positive Airway Pressure) device.

Kamalnayan Bajaj Diabetology Research Centre which is specialised to investigate peculiarities of diabetes in Indians. We described the novel finding that the small looking Indians are thin by BMI criteria but fat when you measure body fat, and that this characteristic is present at birth (Thin-fat Indian baby). Maternal nutritional and metabolic factors which contribute to this are considered. We also described that maternal vitamin B12 deficiency and excess folate levels are related to 'foetal programming' of diabetes. This offers a novel approach to reduce foetal programming of diabetes; a trial is now in progress in the adolescent girls and boys in the villages around Pune. Our research activities fit nicely with the activities of the KEM Hospital Research Centre which was started with the vision of improving the health of the population, especially mother and babies by promoting research and local self-sufficiency.
It is not difficult to see why we believe that the health of young girls determines the health of the nation.

Diabetes Unit Laboratory has over 2000 sq ft. space, with facilities for haematology, biochemistry, endocrinology and related measurements and a substantial computer/data management facility.
All biochemical, metabolic and hormonal assays in diabetes and related metabolic-endocrine conditions are performed in the laboratory of diabetes unit. We have expertise in ELISA, and DELFIA assays, and advanced systems including auto analyzers, cell counters, multilabel-analyzers, and HPLC. Hormonal assays - A hormonal assay test is performed to give an indication of metabolic processes and conditions, or 'hormone imbalance.
ELISA - An enzyme-linked immunosorbent assay, also called ELISA or EIA, is a test that detects and measures antibodies in your blood.
DELFIA - DELFIA® (dissociation-enhanced lanthanide fluorescence immunoassay) is a time-resolved fluorescence (TRF) intensity technology. Assays are designed to detect the presence of a compound or biomolecule using lanthanide chelate labeled reagents, separating unbound reagent using wash steps.
HPLC - HPLC stands for High Performance Liquid Chromatography, and is a technique used to separate different constituents of a compound using high pressure to push solvents through the column. It is the most widely used technique to identify, quantify and separate components of a mixture.
Microbiological assays - Microbial assays or microbiological assays could be a sort of bioassays designed to analyse the compounds or substances that have impact on micro-organisms. They help to estimate concentration and efficiency of antibiotics.
Exosome isolation - Various methods for the isolation of exosomes from biological fluids have been developed. They include centrifugation, chromatography, filtration, polymer-based precipitation and immunological separation.

OPD Laboratory

Important Information:
Diabetes unit OPD lab conducts biological sample collection and blood sugar diagnostic facility for outdoor patients. Trained phlebotomist and nursing staff is available for blood collection.
In diagnostic facility we have Anolox (GM7) glucose analyser which gives immediate and accurate reporting of blood sugar by using 2-3 drops of blood.
